Registered Nurse Navigator Lead

Shift: Full-time/40 hrs per week 8A-5P

Summary:

This position serves to provide clinical oversight and to lead the day to day work flow of the Cancer Center navigation team. Additionally, this position will also navigate patients through their cancer journey. This role will assist the Director in service line development and lead the implementation of multidisciplinary clinics.

  1. Leads the day-to-day activities of the cancer center navigation team, providing oversight to the oncology navigators and other staff as identified by the Director of Oncologyscore
  • Ensures the patient receives the services of a navigator to follow patient through the continuum.

  • Assists with staffing decisions keeping costs and resource utilization in mind

  • Develops a schedule that meets staffing needs based on needs as indicated by disease site and patient volume.

  • Fosters collaborative working relationships with internal and external departments and services.

  • Leads the orientation program for new staff, etc. as assigned

  • Assists the Director in the annual performance review process for the department

  • Assists the Director in corrective action, as needed

  • Assists Director in meeting employee satisfaction goals

  • Assumes responsibility for own personal and professional development and enhancement of skills

  • Participates in project management activities to improve the flow of patient care throughout the Oncology Service Line

  • Ensures the navigators are present at tumor conferences and taking initiative to keep care in the system

  • Coordinates patient care activities and the flow of patient information by effective communication with office staff, physicians, navigators, ancillary health care providers, patients, families, and through appropriate documentation to ensure continuity of patient care

  • Track and process continuing education credits from weekly tumor conference

    1. Provides health care and demonstrates the knowledge and skills necessary for providing care in various settings.
  • Facilitates the navigation of oncology patients

  • Demonstrates critical thinking in prioritizing patient care

  • Develops a plan of care appropriate to patient acuity/complexity and specific to the individual needs of the patient

  • Performs accurate history and physical exams, order tests, procedures, and referrals

  • Schedules the patient for additional appointments/tests/exams as needed

  • Demonstrates ability to assess and anticipate patient needs

  • Provides and discusses treatment options with patients and family members

  • Conducts appropriate and timely documentation of patients concerns, problems and/or symptoms in medical records

  • Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided

  • Ensures that patient care outcomes are clearly documented for all patients

  • Requests written consultation from physicians or other healthcare professionals to ensure continuity and quality patient care

  • Possesses ability to collaborate and communicate effectively with providers, staff and patients to assist in identifying and solving problems

  • Discusses appropriate information regarding patient care with the collaborating or supervising physician

  • Initiates reimbursement for services rendered as appropriate

  • Serves as resource to others in using patient outcomes indicators to organize their plan of care and care delivery methods.

  • Takes an active role in assisting staff to incorporate research and best practices in patient care into their individual practice.

  • Monitors current patient care practices and patient flow to identify inefficiencies and opportunities for improvement

  • Leads facilitation of communication between the Cancer Center Team and the multidisciplinary team

  • Works with providers to implement and leads multidisciplinary clinics

  • Incorporates technology in practice including appropriate use of point-of- care technology and telehealth as applicable.

  • Provides specialized navigation experience acting as a resource and educator to patients, caregivers, and staff

  • Serves as a patient advocate working closely with the navigators and cancer coordinator and team of providers and patient support systems to connect patients with needed services throughout the continuum

  • Works to promote a positive work environment which emphasizes respect for patients, families, physicians, internal and external departments, and staff

  • Collects and documents required outcome metrics

  • Assure accreditation requirements pertaining to oncology services are maintained.

  • Assure grant requirements at met

  • Assists in department specific process improvements to achieve best practices

  • Participates in gathering data on PI initiatives, including chart review

  • Participates in determining root causes

  • Initiates and implements process improvements based on evidence-based practices and best practices in the delivery of care for oncology patients

  • Works to improve patient outcomes through various initiatives, committees and work groups

  • Works with the Director to select metrics to track patient outcomes in each of the care settings/clinics

  • Provides opportunities for staff development and in-service programs

  • Supports the regulatory process by assisting with the inspection preparation and maintenance of standards of care

  • Implements actions to address and improve Patient/Employee satisfaction

    1. Works collaboratively to ensure ongoing program development and improvement within the cancer center.
  • Continually performs environmental scanning for innovations and changes in industry

  • Continually networks with other departments and programs to seek best practice program design innovations

  • Prepares reports for the Director to monitor program outcomes

  • Assists in meeting facilitation and presentation

  • Participates in presentations to spread awareness of oncology services line offerings, both internally and externally

  • Assists the Director of Oncology in the research, development and implementation of disease site specific multidisciplinary clinics

  • Recognizes trends in the delivery of patient care in the department and discusses with Director of Oncology

Education

  • Bachelor's degree in the field of Nursing required

  • Master's Degree in the field(s) of Nursing preferre

  • Work Experience

  • Minimum of 3 years nursing experience; clinical nursing care in oncology, case management or hospice, preferred

  • 1+ years of experience in/with the patient navigation preferred

  • License and Certification:

  • Licensed as Registered Nurse in Illinois required

  • Valid CPR required before start date

  • Certified as APN required within 18 months of hire.

  • Certified by ANCC or AANP or other appropriate certifying body Licensed in Illinois as Nurse Practitioner required within 18 months of hire
