Reports to the Nurse Director. Under the direct supervision of a Registered Nurse, performs according to Department Policies and Procedures, completed competencies, and certification. Functions as a member of the Patient Care Team to provide basic patient care services and procedures to a wide variety of medical and surgical conditions that are admitted/transferred to the Unit.
Meet with assigned Registered Nurse regarding daily assignments. Obtain report from previous shift CNA/Nurse Tech.
Perform basic patient care including, but not limited to, monitor patients’ intake and output, blood pressure, pulse, respiration, temperature, weight and accurately document information into patients’ electronic health record. Assist patients with activities of daily living.
Perform test procedures using Infection Control Standards Of Precaution: venous and capillary blood draws, label and deliver specimens to Lab utilizing approved Hospital procedures.
Stock and maintain unit supplies, patient nourishment floor stock, and obtain daily clean linens, as needed.
Deliver patients’ nourishments and meal trays; provide feeding assistance to patients in need and accurately document patients’ intake.
Transport and assist with lifting and positioning patients according safety and lifting standards. Ambulate patients as needed.
Perform patient comfort measures such as bathing, feeding, making beds, and helping patients into and out of bed in a safe environment and using appropriate lifting techniques.
Perform patient room cleaning; discard all disposable items appropriately, remove linens from bed, chairs, etc. and place directly into hamper and take to Dirty Utility Room. Remove patient care equipment and take to Dirty Utility Room, clean per Infection Control policy. Contact Environmental Services when patient room is ready for deep cleaning and disinfecting.
Assist Unit Secretary in reception/secretarial support with inputting orders, answering telephones, taking messages or direct calls accordingly. Assemble/maintain patients’ charts and perform deficiency analysis. Process forms for patients’ records.
Maintain professional knowledge and skills through self-directed learning and participation in appropriate Continuing Education Programs. Attend mandatory educational programs and inservices as required.
Rotate to other department as needed.
If also functions as a Unit Secretary, will have the following additional duties & responsibilities:
Provides secretarial support for Patient Care Center including but not limited to ordering labs, diagnostic tests, diets, x-rays, along with their audit number written next to the order being processed. Accurately input all patient charges into computer. Perform the necessary computer data entry, copying, and filing (daily lab results, x-ray reports, physician orders, progress notes, etc.) in support of departmental operations. Filing of patient records on charts are initiated by staff member indicating verification of reports/results being placed on correct charts.
Assist in discharge planning (notify appropriate Ancillary departments, copy charts for discharges to nursing homes or skilled nursing facilities). At time of discharge faxes discharge instructions to the primary care physician and any consultants that have requested a copy and complete discharge checklist as items are faxed.
Assemble and maintain patients' chart, including chart order and accurate placement of patient chart forms (under correct colored tabs). Process and adhere patients' labels on all patients' chart forms. Verify Consent for Treatment, Authorization for Release of Information and General Conditions for Admission (CARITA) have been signed by patients of significant others at time of admission.
Request, order, receive and distribute departmental supplies, clerical supplies, and chart forms as appropriate. Oversee the maintenance and servicing of departmental equipment assigned to the Unit. Assist in the containment of hospital costs through conservation of hospital resources. Pick up supplies, equipment or forms from Central Sterile or Warehouse when requested.
Maintain daily census log.
Participates on Hospital Committees as assigned.
High school diploma or GED required.
LICENSURE AND CERTIFICATIONS
Illinois Certified Nurses Aide (certified by IDPH)
Successful completion of Hospital Certified Nursing Assistant Orientation and Training.
Must have ability to read, write, and speak English. Basic reading and math skills. Must have basic computer skills to be able to utilize electronic medical record. Must be familiar with medical terminology, basic patient care skills, knowledge of regulatory standards, safety, and infection control Must learn hospital policies, procedures, and quality improvement principles and tools.
Basic interpersonal skills. Effective Communication and telephone etiquette, and the ability to function productively and cooperatively as part of a team.
Exhibits excellent guest relations to patients, visitors, physicians, and co-workers. Demonstrates courtesy, compassion and respect at all times.
